What services are offered?
Please note: CLR Mind Psychiatry, LLC provides medication management along with supportive therapy. However, if your mental health needs are more complex or require additional support, we may recommend that you work with a licensed therapist in conjunction with your medication management for the most effective care.
Does CLR Mind prescribe medications?
Yes, CLR Mind Psychiatry offers medication management services. Our doctors and clinicians prescribe medications tailored to address specific conditions and symptoms.
For example, we may prescribe medications such as Sertraline to treat anxiety in certain cases. Additionally, if stimulant medications are indicated for the treatment of ADHD, your provider may require testing to confirm the diagnosis before prescribing these medications. This policy helps ensure the appropriate use of medications and supports public safety.
Your provider will work with you to determine the most effective treatment plan based on your individual needs.
What is Gene Sight Testing?
GeneSight Psychotropic is a pharmacogenomic test which means that it analyzes how your genes may affect medication outcomes. The GeneSight test analyzes clinically important genetic variations in your DNA. Results can inform your healthcare provider about how you may break down or respond to certain medications commonly prescribed to treat depression, anxiety, ADHD, and other psychiatric conditions.
The GeneSight test must be ordered by your healthcare provider or nurse practitioner. The test is a simple cheek swab taken in your healthcare provider’s office or can be sent by your healthcare provider to be taken in the convenience of your home.
How soon can I expect a reply from the office or messaging portal?

We strive to respond to most inquiries on the same day. However, please allow 1-2 business days for a reply. If your request involves the completion of paperwork, please note that review and processing may take 5-7 business days.
If you require immediate assistance or are experiencing a crisis, we encourage you to visit your nearest emergency department, crisis center or contact 9-1-1. For urgent mental health concerns, please reach out to your local crisis center for appropriate support also use National Suicide Prevention Lifeline 9-8-8.
Additionally, prior to any session, please complete your paperwork by logging into our secure and HIPAA compliant patient portal (from our partner OptiMantra)

- If this is your first time logging in to the patient portal, please click on the ‘New Sign Up’ button to set your initial password.
- A link will be sent to this email that will allow you to create a password and then sign on to the portal.
- The portal will provide you access to your super-bills, treatment plans, billing information, and more; you will also be able to request future appointments through the portal and communicate with your provider.
After-Hours and Weekend Communication Policy
We are dedicated to providing you with high-quality, responsive care. Our regular office hours are Monday through Thursday, 8:00 AM to 4:30 PM. During these times, we are available to assist you by phone and through the secure Patient Portal for routine matters such as:
Appointment scheduling
Medication refills
Lab or test result questions
General inquiries
Outside of office hours (evenings, weekends, and holidays):
Patient Portal Messages: Messages sent after hours will be reviewed and responded to on the next business day. Please do not use the portal for urgent or time-sensitive medical concerns.
Phone Calls: Calls to our office after hours will be directed to our answering service. If your issue is urgent and cannot wait until the next business day, the on-call provider will be contacted.
Emergencies: If you are experiencing a medical emergency, please call 911 or go to the nearest emergency room or crisis center.
We kindly ask that non-urgent matters be held until regular business hours.

Online vs. In Person Services
Yes, all of our services are available online. We offer the flexibility for clients to choose online visits at any time that suits them.
However, certain services may be more effectively conducted in person depending on the nature of the care required. We encourage you to discuss with your provider which format may be best for your needs.

Who can receive your services?
At CLR Mind, our mission is to provide high-quality mental health care to as many individuals as possible. Currently, we offer services to Tennessee residents. Our comprehensive services include:

Telepsychiatry Available: All services are provided through a secure virtual health platform, allowing convenient access to care from the comfort of your home.
Is medication cost included in the visit cost?
No, the cost of medications is not included in the visit fee. However, the medications we prescribe are typically affordable, and there are two payment options available:
Self-Pay: Most of the medications we prescribe are priced around $10 per prescription. In some cases, the cost may be higher, but we partner with discount programs such as GoodRx to help ensure you receive the best price available.
Insurance Coverage: If you have insurance, we will provide you with a prescription, and your insurance will cover the cost of the medication according to your benefits.

Each initial evaluation is $250 per visit.
Each follow-up is $150 per visit.

Missed Appointment Policy
We understand that unexpected circumstances can arise. If you are unable to attend your scheduled appointment, we kindly ask that you inform our office as soon as possible so we can reschedule. You may cancel or reschedule your appointment up to 48 hours prior to your scheduled time without penalty.
If you do not attend your appointment and fail to notify us within 48 hours, a “no-show” fee will be applied: $75 for missed initial evaluations and $75 for missed follow-up appointments.
If you are more than 10 (ten) minutes late you will have to schedule another appointment at a later date.
We appreciate your understanding and cooperation in helping us manage our scheduling effectively.
Please note: In accordance with our attendance policy, patients who miss three (3) scheduled appointments without adequate notice may be subject to dismissal from the practice. In such cases, we will provide referrals to local providers to ensure continuity of care.
Medication Adherence & Grounds for Dismissal
Medication adherence is taking medications exactly as prescribed. This is essential to the success and safety of your care. Failing to follow your treatment plan can lead to serious health risks and will be addressed as part of your care.
Reasons a patient may be dismissed from the practice include (but are not limited to):
- Repeated nonadherence to medication or treatment plans
- Abusive, threatening, or inappropriate behavior toward staff or other patients
- Missed appointments without appropriate notice (no-shows)
- Failure to complete required documentation or forms
- Misuse or diversion of prescribed medications
- Dishonesty regarding medical history, symptoms, or medication use
- Refusal to engage in recommended treatment despite multiple attempts at support
- Inappropriate use of controlled substances or medication-seeking behavior
Prior to dismissal, we will make reasonable efforts to communicate concerns, provide education, and support your efforts to remain engaged in care. If issues persist, you may be discharged from the practice and provided with 30 days of emergency care coverage and assistance transitioning to a new provider.
Medication Request
At CLR Mind Psychiatry, it is important for patients to attend their scheduled appointments to ensure continuity and quality of care. However, we understand that occasional changes may be necessary. If you need to reschedule, please notify us at least 48 hours in advance of your original appointment time.
Please note the following:
Missed or rescheduled appointments may delay prescription refills. We reserve the right not to refill medications until you have been seen by your provider.
CLR Mind Psychiatry is not responsible or obligated to refill medications if you miss, cancel, or reschedule your appointment.
If your prescription requires prior authorization (PA), please allow up to 10 business days for processing. You will be notified by your pharmacy once it is completed.
Your provider has 48 hours to electronically send prescriptions to your pharmacy. Please check with your pharmacy first before contacting our office regarding prescriptions.

Our clinical team is composed of Dr. Abraham, DNP, APRN, PMHNP-BC who is a board-certified psychiatric nurse mental health practitioner providing care. Our team is highly skilled and trained to provider expert level psychiatry care for a wide range of mental health conditions.
Do you accept walk-in clients?
No Walk-In’s. CLR Mind Psychiatry LLC is open by appointment only and cannot accommodate walk-in patients.
